Was videoing a game from the sidelines last night, and the coach noticed the other team break the offensive huddle with 12 men. Called it out to the ref, who threw the flag and went to the other sideline. Then he came back to the near side and explained to the coach that he was picking up the flag (I was close enough to hear this) because the player "didn't really mean to be in the huddle." Yes, no penalty because "he didn't mean it." How the coach didn't have a stroke right there on the sidelines is what I can't figure out.
